$0 - $50,000                              $100 Annual Membership

$50,001 - $250,000                  $200 Annual Membership

$250,001 - $500,000                $300 Annual Membership

$500,001 - $1,000,000             $500 Annual Membership

$1,000,001 - $3,000,000          $750 Annual Membership

What is a member in good standing?

• Complete an annual survey when requested
• Completed Standards of Care survey
• Provide compliance materials including 501(c)(3) status and W9 if needed
