TCCN has provided funding and provides ongoing support for a robust Community Health Worker (CHW) program. Majority of member organizations have brought CHWs on to their teams in the last several years. These CHWs play a vital role in improving health outcomes for patients by building trusting relationships, connecting patients to community resources, and ensuring patients receive needed care. TCCN provides both virtual and in-person opportunities for CHWs to learn from experts, network with one another, and share their experiences. |

AmeriCorps
In 2024 and 2025, TCCN will engage in planning toward an AmeriCorps program focused on Healthy Futures. We will use this planning grant to address the systemic community issue of access to high-quality, comprehensive, affordable health care, especially for those who are uninsured or underinsured, across the state of Tennessee. AmeriCorps members are well-suited to address the needs of this population as they have a commitment and passion for service to the community that directly aligns with the drive needed to be an effective CHW.
Training & Certification Resources
TCCN works with partner organizations that provide specific CHW training and certifications. They include the TN Community Health Worker Association, the Rural Health Association of TN, Vanderbilt University, and IMPaCT Care.
