TCCN has provided funding and support for a robust Community Health Worker (CHW) program across community and faith-based clinics. Many TCCN member organizations include CHWs as part of their care teams. These CHWs play a vital role in improving health outcomes for patients by building trusting relationships, connecting patients to community resources, and ensuring patients receive needed care. TCCN is pleased to partner with Tennessee Community Health Worker Association to support training and networking opportunities for CHWs in our network. |
