Trusted Voices, Stronger Communities:
The Impact of TCCN’s
​Community Health Worker Initiative

Across Tennessee, CHWs played a vital role in addressing the evolving challenges of the COVID-19 pandemic. From increasing vaccine confidence to expanding access to culturally competent care, CHWs and partner organizations have implemented innovative, patient- centered approaches tailored to the unique needs of their communities. These efforts demonstrate the adaptability, commitment, and long-term value of CHWs in improving public health outcomes.

​TCCN has provided funding and provides ongoing support for a robust Community Health Worker (CHW) program. Majority of member organizations have brought CHWs on to their teams in the last several years. These CHWs 
play a vital role in improving health outcomes for patients by building trusting relationships, connecting patients to community resources, and ensuring patients receive needed care.

TCCN 
provides both virtual and in-person opportunities for CHWs to learn from experts, network with one another, and share their experiences.​

Training & Certification Resources

TCCN works with partner organizations that provide specific CHW training and certifications. They include the TN Community Health Worker Association, the Rural Health Association of TN, Vanderbilt University, and IMPaCT Care. ​
