The Impact of TCCN’s

Across Tennessee, CHWs played a vital role in improving quality of care to reducing health care costs. From increasing vaccine confidence to expanding access to culturally competent care, CHWs and partner organizations have implemented innovative, patient- centered approaches tailored to the unique needs of their communities. These efforts demonstrate the adaptability, commitment, and long-term value of CHWs in improving public health outcomes.

​TCCN has provided funding and support for a robust Community Health Worker (CHW) program across community and faith-based clinics. Many TCCN member organizations include CHWs as part of their care teams. These CHWs play a vital role in improving health outcomes for patients by building trusting relationships, connecting patients to community resources, and ensuring patients receive needed care.

TCCN 
is pleased to partner with Tennessee Community Health Worker Association to support training and networking opportunities for CHWs in our network.​

Training & Certification Resources

TCCN works with partner organizations that provide specific CHW training and certifications. They include the TN Community Health Worker Association, the Rural Health Association of TN, Vanderbilt University, and IMPaCT Care. ​
