In the spring of 2026, Tennessee Department of Health funded a special project to research the challenges that Community and Faith-Based Clinics (CFBs) experience those with diabetes - one of the most significant and persistent public health challenges facing the state of Tennessee.
Fifteen (15) TCCN CFBs participated in a written data survey as well as one of five virtual focus groups. A total of 28 staff and volunteers from our 15 participating clinics shared best practices and explored opportunities for addressing current challenges. They provided input on diabetes management protocols, the most effective strategies to help patients manage diabetes, and the resources needed to improve health outcomes. This report provides evidence-based recommendations for safety net clinics looking to implement proven strategies effective in reducing A1C and as well as strategic state investment opportunities to bridge the gaps identified in this project.
