Conference Overview
TCCN's Annual Conference will focus on "Closing the Gaps" - the challenge of addressing social determinants of health (SDOHs) that contribute to disparities in health and other areas in vulnerable populations. “It’s Now or Never” is a play on the Elvis song of the same name and speaks to the sad truth that these gaps are well known and longstanding. There is a rare co-occurrence of urgency and CDC/TDH grant funds to eliminate disparities at this time. For those in the gap-closing business, it truly is "now or never".

• Three Conference Days -over 27 educational sessions with fresh insights from thought leaders in the Free and Charitable health care sector
